Terms of Service

1. Acceptance of Terms

By accessing or using Creds Clinic (“the Service”), you agree to be bound by these Terms of Service. If you do not agree to these terms, you may not access or use the Service. Your continued use of the Service constitutes acceptance of any updates to these terms.

2. Services

Creds Clinic provides automated provider credentialing services, including but not limited to:

  • NPI auto-fill and CAQH profile management
  • Primary source verification (PSV)
  • Payer enrollment assistance
  • Credentialing status tracking and document management

Services are provided per credential-levels as described on the Pricing page.

3. Account Responsibilities

You are responsible for:

  • Maintaining the confidentiality of your account credentials and restricting access to authorized personnel.
  • Ensuring that all provider credentials, attestations, and supporting documentation submitted through the platform are accurate and complete.
  • Notifying Creds immediately of any unauthorized use of your account.

4. Payment Terms

  • Credential credits are purchased upfront prior to initiating credentialing.
  • Credits are non-refundable once primary source verification has begun for a provider.
  • Unused credits may be applied to another provider within your organization at any time.
  • All fees are stated in US dollars and are exclusive of applicable taxes.

5. Data Accuracy

You are solely responsible for the accuracy, completeness, and truthfulness of all information submitted for credentialing. Creds performs primary source verification (PSV) but is not liable for delays, denials, or adverse outcomes caused by inaccurate, incomplete, or fraudulent data provided by you or your organization.

6. Limitation of Liability

Creds facilitates and expedites the credentialing process but does not guarantee approval by any payer, hospital, or credentialing body. Decisions on provider enrollment and privileging are made solely by the respective organizations. To the fullest extent permitted by law, Creds shall not be liable for indirect, incidental, or consequential damages arising from your use of the Service.

7. Termination

Either party may terminate this agreement at any time. Upon termination:

  • Unused credential credits remain valid for 12 months from the termination date.
  • You will have 30 days to export and download your provider data before it is removed from our systems.

8. Governing Law

These Terms of Service are governed by and construed in accordance with the laws of the State of Delaware, without regard to its conflict of law provisions. For legal inquiries, contact us at legal@credsy.clinic.
